criteria for BCBS of MI

Thanks so much for the letters! Any suggestions for me since I just changed providers in November? I am thinking about just filling your letters out with my info and seeing if the new PCP will sign it, but what should i put on how long I have been his patient? Or leave that part out? I do know that he is supportive of me having the surgery. My last PCP would not even talk to me about it and is why I switched. Thanks again, Shonna
