Question for Henry Ford Patients

I received my white folder from HF that they send you once you are approved and set up for your nutritional consult. In it there is a paper that described my phone converstaion with Shantel and my approval. It also states that at this time a copy of the patients history and physical is needed (this is sent to my PCP I'm assuming) and that I was instructed to hand deliver this (which was never mentioned). My question is.. do I need to contact my PCP about this? I already have a copy of all my medical records and I had my last physical in October, which was done by my other PCP (however it is mentioned in the records). I guess I just don't know what they want... can anybody shed some light on this for me?

Actually, I looked through my materials a little better and it states that eithier the history OR a letter from you PCP stating the need for surgery. I have my referral letter, so hopefully that will do the trick.

I so need to get approved. I feel like I wanna play the game but the coach won't let me in yet. Unfortunately, even if I get approved on my appeal, I doubt I can get my psych eval in time to make the May 2nd seminar. Sigh. Im still rooting for ya, though!

Kim, I had my surgery at Henry Ford in January and just love Dr. Genaw. He is very understanding and encouraging. Do you know yet what Doctor you will have? Best of luck, I never knew my quality of life could change in just 3 months . . . it's great on this side! Kristy

You actually don't need to turn in anything at the nutritional consult (well, $200, but you're HAP right?, so then the $200 is covered). Nothing was probably sent to your PCP. You might want to get a copy from your last physical that was done in October. As long as you have the other records, you should be fine. Take care, Nicole

Really?!? I thought it did. I've attended the 4-hour consult meeting 4 times and I could have sworn that Wanda said once you have HAP approval that the $200 is covered. Hm...must have changed! Bummer!

My packet states that the consult IS covered. The only thing not covered is the co-pay for the hospital which I believe is $1,000 - I'll have to call HAP to verify. They sent me the guidelines on what HAP covers and the Nutrition/Diet Consultation is included as well as the Inititial Surgeon Consultation and the pre-surgery consultation with him, 2 months of visits after surgery, blood work and stress test... All covered.

OK..that's what I thought. I knew that if you have HAP you can't even attend the 4-hour consult until you are approved. From what I remember you're supposed to have some sort of letter to show that you're approved, thus covering the $200 for the 4-hour class. I don't have HAP so don't quote me on that...I just remember that at my class I had to write a check for $200 and was jealous of the HAP people who didn't have to write a check! Hi Kim, You don't need anything for the N/N consult. When you see your surgeon for the first time you will need the papers they sent you(in the white folder) filled out. I don't remember what all was in there, except for diet history. You will need your letter of intent to pursue gastric by pass surgery, your medical records-which should have your last physical in them. You may want to take your letter of necessity from your p.c.p.. They will have you write down every thing you put in your mouth the day before your appointment, even if it was just a taste, but they don't tell you that until you get there. Hope this helps.
