I went to Dr. Ellen and they have you fill-out paperwork, look at a book of peoples before and after pictures, talk to the PA about your expectations and then the doctor comes in to examine you. She discusses what she thinks she would need to do to get as close to what you want as possible and also if she thinks there is potential for "dogears" and how they are handled. I thought she was very thorough. If you decided that you want to go forward then they take "before" pictures to use as documentation for the insurance company. If you decide not to go forward they have a out of pocket charge for the consult. I never had to do anything to get approved. They called my insurance company (BC/BS PPO) and were told that if I met the criteria I could have the surgery and did not need to be pre-authorized so they scheduled my date the next morning.
