Does anyone have a current copy

Hi Colleen you will go to the hap.org website enter your user ID and password on the left side of the page click on contracts and riders click your name click HMO subscriber contract hope that helps.... Sandy

Well Sandy, I did that. Its the same damn thing they sent us in the mail. I wanted the list of specific criteria that has to be met. ya know, the stuff that says you must have 12 months medically supervised, BMI, Co-morbids, etc Oh, and its about time you started posting!

I dont think Ive seen anything with the actual criteria........if you find it let me know...... Only reason I never posted was that I really didnt have any answers..lol
