Anyone had experience with PHP State of Michigan plan

Hi, I am going through the hoop jumping with BCBS right now. My issue is that I have the opportunity to switch over to PHP State of Michigan Insurance but I have no idea what their requirements are for surgery approval. Do they require the 12th month consecutive weight loss documentation that BCBS does? Any information would be greatly appreciated! BTW - I have to make this decision by early next week!
