If you are referring to BCBS-Community Blue PPO, what I did for my surgeon, was write down my past history of dieting failures and success stories from early on to present day and that was suffient enough for him and BCBS -CB-PPO. Now I would imagine if you really want to know, I would advise you go to the source and ask them. But my PCP suggested the idea and it worked for me. Who knows, maybe it'd work for you, Good luck Stacey

I just completed my year of dr. supervised diets. My dr. could not honestly say I had made attempts UNTIL we noted it on my chart, meaning weights, discussing diet and exercise. I opted not to try any diet pills. I had to do the full year, but it goes by faster than you think... My date is Feb. 6th. Good luck. Tiffany

I don't know how much BCBS-PPO has changed, but when I did my diet history I wrote my previous attempts down and gave them to Cori Center myself. I know my PCP was never involved with the approval process or asked for records, because I applied and had surgery within a month and didn't see my PCP until after it was done and he was surprised. Cori took care of everything for me, but I really don't think they had much problem getting it approved.

Granted, I had my surgery two years ago, things may have changed alot, but I made copies of my weigh****cher weigh-ins (a years worth), wrote down every diet I tried, wrote a letter as well expressing my reasons, and I had no problem getting approved. I'd say just call the number on the back of your card...see if it's covered. Best of luck to you! Theresa
