I am very intersted in getting lap-band surgery. I already went to an informational seminar and am scheduled for my medical evaluation. I have have verified with my insurancane company (BCBSM Community Blue PPO) that the surgery is covered and no pre-auth is required. However, they indicated that I need 12 consecutive months of physician or professional supervised attempts to lose weight. Problem is, I don't have it. I have been over weight my entire life (30 years) and within the last 7 years it has increase to morbidly obese (BMI 40) I have enrolled in The Medical Weight Loss Clinic 2x during this time as well as had physician prescribed weight loss pills such as Merdia and Phentermine. But it has not been "physican or professional supervised" for 12 consecutive month, although I did continue with them for over 12 mo, just not supervise. (became to expensive). Anyway has anyone been approved by bcbsm without having this documentation?(consecutive 12 mo supervised)

I would talk to your doc and get what documentation he/she does have. Then get the records from medical weight loss and see how much is there. who is your pcp? with my insurance, i just told them it wasn't my fault my pcp didn't document it. I would try it anyway, and in the meantime, let your pcp put you on the diet and do the weigh ins how often you need them. I am also in Pontiac, if you need help let me know Hugs Linda

Thanks for the reply. Here is the thing. I have not seen my PCP in about 5 or 6 years. However, the PCP I had, I had since since childhood. It was with him that I was prescribed my first weight loss medication (Merida). I don't have any reason as to why I stopped seeing him. He is still my families PCP (my brother and parents) and is pretty close with them. His name is Dr. Haber. Any illness I have had over the years I just went to urgent care and I was given the names of a couple other physicians who I went to soley for the other weight loss meds that I have taken over the years. Thanks for the support. Brown eyes

I have BCBS PPO and I also had to have the 12 month consecutive diet. In around my 7th or 8th month my doctor submitted for early approval and BCBS was not having it so I finished out my 12 months. They don't budge from my experience. Good luck!
