insurance concerns

has anyone else had trouble with a provider saying your coverage has changed and you are no longer covered for any type of obesity treatments? i called trying to get pre approval for the nutritionall consult and they stated that my coverage had changed the month befor and i was no longer eligable for any kind of obesity treatments. i had not been pre approved prior to this and i was wondering if anyone else has gone through the same kind of thing with their insurers? good to know others out there are getting approved. it makes me think there is still hope for me. concered.
