Calling Toder patients

Hello all, I had my surgery in June and lost my insurance in September so i haven't had any follow up since then, i need to get back in to see Toder and the NUT's and have bloodwork done. I just got back insurance  but  I lost my binder and was wondering if someone could help me out with the phone numbers for them. Thanks in advance

I had Laproscopic Bypass with Dr. Toder on June 12, 2008, too. I don't have my information handy, I had moved in August 2008, I have no idea where my information is.
