Well good grief. I am so sorry I had to come up with 5 years of weight documentation. Only reason I knew ahead of time is I hounded the crap out of the office. I tried to get my surgery in December just didn't quite make it. I would call WW again and explain your situation to them. If the person you speak to is not helpful ask if there is someone else you can speak to. If it is somewhere local the information is kept maybe you can go retrieve the info yourself. Hope you are able to get the info you need soon.
I was told up front that my insurance usually wants a 5 year history of weights too. Luckily, I've been seeing the same PCP for that whole time and they weigh me everytime I go...UGH!! Hopefully your work records will work. It's amazing the hoops we have to jump through just to get an insurance approval!!

My insurance wouldn't accept WW records. For my 6 month diet with my PCP I did WW but also went in and saw her every week / two weeks and got weighed in by her. As far as diet records, even if you haven't had the same PCP you should be able to get records from any doctors you have seen over the past few years.
