NEW to this, I need Insurance help

I am just looking into this whole lapband thing because a friend of mine had it and has had great results. I was wondering if you guys had insurance companies that approved it right off? I have Anthem BCBS. Right now Im 5'2 weigh 240 and am hating it. I have a underactive thyroid, type 2 diabetes, anxiety/depression and haven't been able to lose weight. The most I have lost has been 40 lbs and it has come right back with little to no change in my diet and exercise. Im going to approach my doctor about it and I think I should gather all sorts of stuff for her. I have mapped out my blood sugars for 2 months, my daily diet for 2 months, my activity level and exercise for 2 months and haven't gained anything. Anyone have advice?
