I have NCAS but that company is just an administrator of my employer's health plan. So it would depend on your employer's health plan I guess as far as approvals and requirements. For my surgery approval, they required 6 months of supervised weight loss or weight loss attempts. My primary care doctor never wrote much in her notes about weight loss despite that fact it was always a topic. So I got her to write a letter stating she has been following me for x number of years- suggestiong various diet and exercise programs (gave few examples). She also stated she recommends this surgry for me due to lack of sufficient weight loss via traditional means and the fact I had a few co-morbidities. Depending on your surgeon's financial/insurance staff- i would contact NCAS yourself because I ended up doing that and got the approval thru much faster. It seems the surgeon's office or the financial person had loads of tasks each day and my paperwork was not her priority. So I ended up getting the letter/info from my primary doctor and faxing it to NCAS myself. That got me approved much faster than waiting for surgeon's staff to do it. I wonder now if they space out the submission of materials for the approval becasue they may be back-logged with needing to schedule surgery dates (or they may have wanted to collect revenue for their diietician for the 6 months sup. wt. loss when I technically had that requirement already completed). If I hadn't asserted myself and gotten involved with NCAS directly, I imagine it would have been at least 6 months before I ever got the surgery. I might still be pre-op now if I hadn't taken control of my situation. Good Luck Karen

I have NCAS insurance. I had no problems with them at all. I got my approval in three weeks. I am having my surgery at Union Memorial Hospital and they took care of everything. My primary care doctor wrote a letter to NCAS six months ago when I started the program at Union Memorial. Maybe thats why I had a quick response. Good Luck Kelly M.
