Did you have a program fee...and a couple other questions

If you all dont mind sharing the info. Im tring to make a list. I could even make a spreadsheet to share with newbies. It would be nice to have one sheet to look at instead of trying to reasearch over and over when I think of something else.

What Im wondering is...

1. Who was your surgeon?
2. What surgery did you have?
3. What hospital?
4. Did you have a program fee and if so how much?
5. What insurance?

I figure theres plenty of Marylanders who went either MD, VA or DC

Here is my info. My surgeon did a great job...no issues with him, just the band that doesnt agree with me.

1. I met with Dr. Halmi, but Dr. Tran actually did my surgery
2. Realize band
3. Potomac Hospital
4. I was self pay and for that, they had $4000 I think as the fee. Not sure if you pay that with insurance.
5. I was self pay at the time (apx $18k) Now I have BC/BS federal and they have been covering my recent fill/unfill and office visit.

1.  Dr. Garber.
2.  RNY.
3.  I had  my surgery at York Hospital in York, Pennsylvania.
4.  Highmark Blue Shield Classic Blue.
5.  I paid 20% co-pay.
6.  The weight loss center charged $15/month for their classes.  This was a requirement of my insurance company to attend 6 months of weight loss classes.

I'm very happy with my outcome thus far.  No more medications, no more health issues, and down 84 pounds too!

My surgeon was and is David von Rueden at St Agnes Hospital
I had the RNY surgery at St Agnes Hospital
Our program fee is $750 per person.  BUT that covers:  all NUT and PSYCH meetings before and for a year after surgery
Our insurance is FEDERAL blue cross and blue shield.

I don't think it's the hospital's fee.  Surgeons set their own fees.
