Do you know how detailed the diet noted from the physician must be and exactly what tupe of information the insurance is looking to see?
            Highest weight- 325  Day of surgery- 301  Post surgery -306

your surgeon will have a form you can give to the previous place.  Unless its weigh****chers, you just give your litte weight card....
