It all depends on your insurance policy.
My difficulty was that my insurance company told me i did not need a 6 month supervised diet; however, when it came time to submit to insurance and I asked for that in writing they would not provide it. I went to my HR department and found out that my insurance really DID require the 6 month supervised diet.
