2. NO requirements for length of time obese
3. NO supervised diet prior to approval.
4. ALL expenses including psych and diagnostics prior to surgery are covered under plan specs.
5. We owe 15% of the surgery, the anesthesia and other doctors and $200 per hospital visit

One of the reasons why I've always wanted a federal job!!!!!! I just couldn't wait any longer and went through my employer's insurance to get mine.
But that doesn't mean that I'm not going to ever work for the Fed. I still want to! :)
When I get done with my MBA, and the staffing at my current job doesn't change when a particular someone special retires, I'll be looking to get in Federal because I wouldn't be getting anywhere staying at my current job.
May want to keep a seat dusted off over there for me in a year! LOL!
But that doesn't mean that I'm not going to ever work for the Fed. I still want to! :)

I am here nearly 20 years.
I earn 4 hours of sick leave per every two weeks
(and I can have up to 240 hours of it ADVANCED if need be)
I earn 8 hours of vacation leave every 2 weeks (26 days per year)
I can also work over and get credit and comp (so i work 8 hours on a saturday and have 8 hours of leave immediately available)
when i have to leave (sick kid, sick dog, broken husband) I go. no questions asked. no problems.
i'm wearing jeans and a long sleeve teeshirt today. (as I am on most days)
and i'm surfing the net!

You are very lucky. Many have to struggle and fight with insurance. I have the same Fed BCBS, however it wasn't completely fee free for me. I still had copayments to make. My hospital stay cost 100 bucks, guess it went up after the 1st of Jan. Oh, and BTW, those last two statements are what give Fed workers a bad rep. Now get back to work!!!

as for jeans and teeshirts... i'm a computer geek... i am supposed to surf the net... LOL

Sounds like hubby... LOL And he isn't even a fed yet... just a contractor... Though his "time off" isn't as good... 6.67 hours total sick and vacation for each two weeks. He gets paged at least once evey week, and get an hour of comp time - checks email and answers it from home, another hour of comp... it adds up really quick sometimes! Jeans are a daily thing... does USUALLY do a button down shirt LOL
As for my surgery on their insurance - we have Guardian LIfe PHCS Multiplan - at first I thought it sucked but I have learned just how wrong I am... I paid $150 for a psych eval, $150 for the NUT visit and will pay $35 for a blood thinner after I am released from the hospital, that is all... I do have a $250 deductible every year, but I have already met it....
