Within each insurance carrier, there are so many diffiernt plans, ihat it is really hard to compare. I do have BCBS. I was required to meet the 6 month documented weight loss attempt. I was given credit for 3 months because of documentation I had using NutraSystem the year before. Once I was submitted, I was approved fairly quick. In my particulat case, that original approval expired, because I ran into a medical complicaiton that had to heal first. When we went through the re-approval process, there was a mishandling of my paperwork which delayed things a bit. Once we got the paperwork thing straightened out, things were approved quickly.
Regardless of what insurance you have, the most important person to keep in touch with is the financial services person at your surgeons office. Get to know them very well and keep in touch with them once you know your paperwork is ready to be submitted or has been submitted.

Debbie is right - most policies are different based on the riders, exclusions and specifics that a company has put into it.
Fortunately for me, I had BCBS Federal like JillyBear so I encountered no trouble at all. The 6 month requirement did not apply and my approval was back within 4 days of the office submitting the paperwork.
No matter what - don't give up! If you begin to run into road blocks, just remember how much you want this surgergy. Best of luck to you!
