Teresa, I have BCBS also, and the only requirement they have is a six month medicaly supervised diet.  And of course her co-morbitities will help with the approval also.

Whether or not WLS is included in her employer's policy is also something she needs to check.  The guidelines may also be governed by the employer's policy. Mo

Comparison is the thief of joy!

If we spend our time comparing our life/weight loss/body to others, we totally miss what WE have accomplished.   Keep in mind how far you have come and what you can do now that you couldn't do weeks/months/years ago.   I hate the expression " It's all good", but in this case it fits!   Wherever you are in your journey  ... It's ALL good!!!

Call the customer service number on the insurance card and ask them if you are covered.  If they ask for a "number"  tell them - laparoscopic gastric banding ( #43770), diagnosis code for obesity is 278.01 Hope this Helps Gayle

Like the previous posts say, call insurance company and define your own plan and ask them to guide you through the SPD pages (it is usaully a PDF file updating the benefits and plkan specifics every year) and when they guided you to that specifc link, search for Morbid obesity, Weigh loss Surgery, The code mentioned above or any of these metadata and you will find your employer's inclujsion or exclusion for this procedure. Every employer is different Ellie
