I wear a bracelet from www.medicalert.org . You pay an annual membership fee, but in addition to the bracelet you can have your history and/or contact information on file and that can be accessed via phone by emergency personnel.
I talked to my surgeon about a bracelet and he didn't really see a need for it, but when I explained to him that I spend a lot of time on the road alone (for example my Easter weekend jaunt to CT) and if something were to happen to me, I would feel more secure.
When I placed my order with Medic Alert, I didn't specify the wording to be engraved and then worked with one of their customer service reps who was very familiar with WLS and made suggestions about what to have on the bracelet.
I have had many friends who are in emergency services type work caution me about having a piece of 'jewelry' for a medic alert bracelet. If it looks like jewelry it may well be missed. My bracelet is exactly what it is .. a medical alert .. I don't want anyone to miss it if I need it.

Hi, I make jewelry and have made several medic bracelets for children with diabetes (for free). I'd be happy to make you one; I only use real stones and Swarovski crystals and SS and GF, no plate or nickel (a common allergy). Plus, I won't charge an outrageous fee, just the cost of materials. It may be the metal of the plate which is bothering you -- you may have to spring for a gold one or you can also coat the back with some clear polish so the metal doesn't touch your skin.
