Dr. DVR doesn't perfomr the surgery on those 18 or younger. Heard him say it at a seminar once.

Generally the place to start is with her primary care physician. Since you want to have insurance cover the surgery, the PCP needs to give you a referral in many cases. And the PCP needs to document previous diet attempts, etc. That said, I recommend Dr. Li at Sinai.
