help please!!!!

hi everyone. i need to know how to get the papers i need to fill out for my first session with dvr. i was supposed to be sent some info but haven't gotten it as of yet. the only thing i have gotten was some info on washington hospital and some of the doctors that do the surgery there. nothing to fill out and i do believe dvr in baltimore (correct) ?? if anyone can give me some info i would sure appreciate it. also glad to hear everyone is getting better from all these germs. maybe all that snow and i ice we had killed some of them nasty things that were flying around in the air. anyway everyone have a good time this weekend. and take care. tammy

Hi Tammy, I don't have any info on DVR but there are several on here that are his patients so I am sure you will get a response from one of them. I would suggest that you call DVR's office and ask about having them resent. Hugs, Mo

Tammy, call his office --443-849-3779. I waited for my papers to go to the information session too. Finally, I called the office and she set me up to be at the meeting. She then sent my papers. Originally I had signed up online and I don't know if they got that or not. Good luck...Gail

Tammy, After you fax the documents, follow up with a phone call to make sure they received them. If the documents are Word documents sned them attached via email. Come to think of it, you might be submitting the form directly on line.
