Make sure your surgeon's office follows up with them in a couple days. Optimum Choice actually approved me but forgeot to phone the surgeon's office and the mailed letter of approval was received a whopping 20 days after approval The approval had a 30 day expiration so I ended up having to apply for a 30 day extentsion on my surgery date. No biggie, it worked out but later I found out that they OFTEN approve you and don't call to tell anyone, so have your surgeon call to follow up with them. Good Luck, Roni
