okay, so....

Okay, so I am thinking I want to ask my Dr to refer me to Edmonton when I see him next month.
Now, where do I find  paper work or referal forms for him? or what do i give him for info so that he can refer me?

Your Dr. will have it.  It is just a letter they send with your basic medical history, weight and BMI.

 my gp new nothing about edmonton. and he never mentioned the wpg program to me. my psychiatrist new about the wpg program. but it wasnt until i was referred to my endocrinologist dr h lee that i got refered to edmonton. and hes very familar with the program, and will follow up with me for life. he will also be doing my follow up labs.
if your dr doesnt know what to do pm me and ill give you my endos contact info.
