Its the St. Boniface Surgical Associates on Tache Ave. I dont have a complete address handy but your doctor would be able to find it quite easily. I think the criteria is women only, under age 55 bmi under 47 (not 100% sure on that). Although the phone message says 18 months waiting period, its apparently more like 3-5 years now as one of the surgeons went on mat leave. Wish we could get a clearer time frame on the weight period.
