Denied by Aetna Insurance :-(

My partner was denied by her insurance for lack of documentation. We are having her doctor write up a letter but this is the first time her doctor has ever had to write a letter like this. What exactly should be in the letter? Has anyone ever had this happen before? New to all this so any help would be great. Thank you

Usually the insurance companies want to see documentation from your primary care doctor documenting that you have tried to lose weight over a period of time (from my experience it is usually a five year period). Have him list the diet plans you have tried and the weight you lost with each. Good luck!
