That's frustrating- but it's also your responsiblity alone to follow up on your one year/ two year, etc. check ups.
Your records are safe at the hospital where you had your surgery. Surgeons who quit do not take the records with them. I'd follow up with them & get copies for yourself.

To be honest, I've never known a practice to let the patients know when there is a change in staffing. My surgeons office has added doctors & one left since I had my surgery & I only know this because of the changes I see on their website.
