lapband to revision question/ help please

 I had lapband surgery in dec 2005. My current issue is that i sat in the red zone for so long i have created a 2nd pouch and currently have no restriction. I am 5'2" tall and currently 171. I have gained back 30 pounds and never made it to goal. I have many issues from the fact that I was in a car accident when I was a child and had part of my colon and intestines removed. I have had numerous surgeries, 3 hernia, hysterectomy, and intestinal blockage. I have so much scar tissue that my skin is so tight on my stomach that it is pulling apart. For the past year my doctor said he can not make the band any tighter and yet I have no restriction. I have several questions about what my options might be and have a consult with my surgeon this week to discuss. A friend suggested that i get on this forum and ask some questions so here I am. Please see below:
1.) revision to plication or vsg?
2.) Why do they keep the band in when they do plication? 
3.) what happens to the stomach that they fold over? How can the organ still stay vital and not die?
4.) Can I have plication without removal of the band?
5.) with my current condition how bad the scar tissue is from having the band in so long and so many previous surgeries, is this a risk of additional scar tissue and causing blockage in the future.
6.) where can I get more information on plication?
7.) why do so many band patients elect to do plication over the sleeve?
8.) I do not know why my surgeon has not suggested this option before and why i had to bring it up.
9.) do i have a chance for approval with my lower bmi even though i am having so many current issues.
I know this is long but I am so frustrated with my weight gain and am afraid that i will continue to regain if nothing is done.
