What can I do, besides/along with plastic surgery (if need be), could do to help my skin go along with the sagging that I am worried about from extreme weight loss?

I think that it varies depending on genetics, if you smoke and drink alcohol, and if you exercise, what kind of exercise and whether or not you have taken care of your skin in the past.  I have lost 90 lbs and I have some sagging skin issues.  I moisturize daily and I do a bunch of resistance training and I walk/jog as well.  I will be having some surgeries to remove sagging skin around my tummy, and thighs as well as a breast reduction.

cocoa butter! Since my surgery, I am using every day cocoa butter- it's supposed to help a little toning the skin, plus it smells nice :)

Personally, I don't think anything can be done short of plastic surgery. Once the elactitisy of your skin is gone, it's gone. It all depends on these like genetics, if you've been pregnant, how many times you've lost and gained weight, age.
I have had a tummy tuck/boob job and a face lift. I am very happy with the results.

I have some saggy skin on my inner thighs, butt creases and lower tummy.  I have worked out hard and religiously for 2 years...and unfortunately *for me*, no amount of exercise or specialty lotions have completely rid me of these issues.  It has helped the appearance, however - at least there is muscle tone around the saggy skin to thwart people's attention from the sagginess.  LOL

I have come to terms (it took a WHILE for this) with the fact that I will never be perfect and I will take saggy skin and a healthy lifestyle over my 259 lb morbidly obese body any day of the week! You are going to be fine and hopefully you will just love your new body and the skin it's in. In due time... :)
