Have you been in to see your surgeon recently? If not then that is the place to start. You need to have your fill level checked, see the nutritionist, get your labs evaluated, etc. You should start over as a newbie. Find out what they recommend and follow the plan. Get in to some support groups. If you need to see a counselor familiar with eating problems, do so.
You are not in an impossible position, nor have you failed. You have just taken a side path and need to get back on the proper road. I would highly recommend buying a copy of Jean M.'s book "On The Bandwagon" and suscribing to her free e-mail bi-weekly support articles. You need to be aware that not only have you probably forgotten much of what you learned before, but things have changed drastically since you had your surgery.
The beauty of the band is that we can start over and succeed. You can do this, good luck!

Call the hospital where you had your surgery and see if they know who took over his practice.
Doctor's don't usually just "close their doors", they sell the practice to another physician, and your records would be with whoever bought it.
Or call the Harris county Medical Society. They might know. (I don't remember if Humble is in Harris county or not.. if not, call your counties medical society)
If all else fails, there are lots of bariatric surgeons in the Houston area. I'm sure one would be happy to have a new patient. :)
