Wow.  Is the whiteish/yellowish stuff body fat?  And what is the thing hanging down from the throat area that looks like a uvula?  Is that the stomach?

I think it is fat.

The uvula shaped thing I bet is the heart.

I find the digestive system the most interesting...look how much bigger it is in the 250 lb person.  The heart is enlarged.  And the bones in the lower legs of the 250 lb person look thinner and not as bright white...possibly meaning they are weaker???

the bones are also slightly bowed.. esp. the ankles.

I've always been amazed at what carrying that extra weight does to our knees and legs and overall structure of our legs.  When I watch the Biggest Loser it's one of the most striking things I notice; that their legs look knock kneed like they're colapsing.  This clearly shows why that is and it seems to start at the ankles.  When the ankles turn out the knees go in.

I keep coming back to this post.  This journey REALLY IS about what you look like on the inside...not the outside.  And this picutre really brings that home for me.
