my doctors office already faxed it.They did it the same day.But the paper given to me by Dr.Belloews office said i needed my medical history and a medical release from my PCP.My question is does the letter he wrote consist of both.

I went to see my PCP.i asked him for the letter need for clearance to have wls.he pulled out a script pad and wrote a quick note.It did include my dieting history and how long i remained on one under his supervision.It just seemed so not professional to me.but then again I'm not a doctor maybe that's what they do.Plus i would have taken it on a napkin if need be LOL!  Just wondering about that.
