first appt. with pcp

My first appt. with my pcp is tomorrow.What should I make sure is noted in my chart for my supervised diet? Is there anything I need to make sure is always written each month on my chart?Thanks Teesha

Teesha, I am not sure what you need. I did not need a supervised diet but my PCP had always documented my diet attempts since the first time I saw him. I was amazed at the how he really paid attention for all that I said for the past five years. Good luck tomorrow and keep us posted. Jodi
