How do I get started

Hello,everyone.Im trying to get this referral on the way does anyone know what has to be on the referral,cause I called the nurse,and she is playing dumb founded.I really need this surgery.Have anyone used transportation from medicaid  services?The doc is located in new orleans,Im here in lafayette.If theres anyone that can help me,I really appreciate it.I lost my mother last year and just been going through alot..God bless yall..... Hello, what type of referral are you speaking of?? If you are talking about a Medicaid Referral just have the doctor state for Bariatric Consult due to...(whatever your diagnosis are). Usually the doctor will refer for Diabetes, Hypertension, etc, to be evaluated by the Bariatric Surgeon and will list your BMI.
As for transportation services, call your local Medicaid office and they can give you the 1800 number for medicaid transportation.

Did you find a Doctor that will accept Medicaid?  What Doctor?  In New Orleans?
