Oh let me add, before my appointment I had sent a referral letter from a doctor, i had all of my records from my doctor's faxed over, got the record from Dr. Tracey....all of my doctors usually weigh me...and then I had a nutritional assessment from my coworker that is a registered nutrition....So I started the ball rolling. I hope it is enough. My PCP sent 5 years and also my Cardiologist.....so I cant see me NOT having enough info....
It sounds like you are in good shape. If you had 5 years from your PCP, then you are in good shape. I was missing 2 years, not consecutively, because I hadn't seen a Dr; I wasn't sick. But March 2007 I was diagnosed with Diabetes so for 2007-2008 I had lots of stuff! I had to go and pull records from Dr. Tracy, a Dr. in Kirbyville (all diet Drs.) and my gynocologist. The hardest part for me was the 5 year history. And I only had 6 months of Dr. Tracy stuff, but that's all they required. Incidently, I went to my PCP today,and my diabetes #'s are all normal, and I've taken no meds since surgery 7/3. He pulled me off of all diabetes meds, blood pressure meds, and cholesteral meds. That in itself is worth it all. Oh yea, the psyc eval was done on Thursday, and Dr. Chung had those results by Tuesday. I wish you the best of luck. Please keep me informed of your progress.
If you don't mind, please send me an e-mail and let me know who you are and where you are from. My profile says it all. I would love to share tips with you and encourage you through the surgery. I know you are going to do great. I promise I will be totally honest about the good as well as the bad. Right now I am 4 weeks out and feel like my old self again. I am now eating anything I can tolerate and I do have to remind myself that I have had surgery. AS they tell you all the way through, this is a tool, not a cure all. You still have to work at it. But it does make it a whole lot easier.
I sent you a personal Email on the 28th and I sent one today............and please share all the tips...I am anxious....I have my psych eval on August 14th and I guess I am just ready for the doctor to say "I have a surgery date for you." Then I will know that I am on my way.
I do understand about the tool....I just figure if I get a real fresh start, train my mind, I can be successful. It never hurts to try and that is what we are doing...Can I ask, did you have to pay a lot out of pocket?
