Hey Jessica, Keep on fighting with the SSI. They always turn people down the first time, it may even take you 3 times before you get it. After so many you'll have to go in front of the administrative law judge and get a decision that way but just stick with it. call dr Prater's office at pathways and they will work out a payment plan for you. That questionaire is simple and it won't take you very long. I told him that I talked to lots of people on here about surgery and that helped alot. He's just mainly wanting to make sure you know what your getting into with this surgery. He's really nice so you won't be nervous talking to him. Have you seen Dr Nease yet? I dont know if medcaid still covers the evaluations at KRCC ( Kentucky River Community Care Center) in Hazard. ( Perry Co) In 2002 that is where I had my evaluation done. It took me about 2 months to get in because they kept cancelling appointments and it was very frustrating but I stayed on top of it until they finally did one. I only had to pay $5. It took about a week for the report to come back. They ask you crazy questions over and over. I had to do a 500 question test and it was basically about 10 questions asked in differnt forms. It took about 45 minutes to do all the tests. I dont know of any other place that does them. KRCC works on a sliding scale, so even if they dont take the medical card it wont be $140 for the evaluation. Give them a call and see if they will see you. The phone number is 606-436-6761 . The dr I saw was Dr Dummer ( pronounced Doomer...LOL I know it looks like dr Dumber !!!)
