Insurance question

Can anyone tell me what requirements were/are needed to get approved for this surgery with Humana ChoiceCare? I would like to have everything ready to go when it's time, but have no idea what I'll need.

Linda, I don't know about Humana, but my insurance (BCBS) required 6 months documented weightloss attempts (dr, weigh****chers, etc), a pshyc eval (scheduled with surgeons office), a sleep study to verify/deny sleep apnea - a comorbidity (scheduled with surgeons office), evidence of co-morbidities - I submitted 10 years worth of doctors office visitation notes, this documented my high blood pressure, diet attempts, depression, severe gerd, I also submitted a personal letter, and a letter of support from my pcp. Oh, I also had a nutritional eval (scheduled through surgeons office) and I'm sure the surgeon also submitted a letter for qualificaitons summarizing my doctors notes, and my personal letter. I can't think of anything else, but I also submitted my ob/gyn office notes, from having two children and dealing with weight issues...just anything you can think of, especially if you've been on medicine or under doctor care to try and loose weight, this is the most important...it took me about 3 months to get everything together, including the appts scheduled through my surgeons office. Once everything was faxed and offically recieved they had a decision in 24 hrs! I couldn't believe it! I was ready to fight, but I got all worked up for nothing...I hope you have the best of luck in getting a quick speedy approval! Hope this helps, email me with any questions! Michelle

I have Humana PPO so I don't know specifically about the Choice Care but they did not require the 6 month diet. If you have a BMI of 40 or greater or 35 with co-morbidities that's all they require.

Unfortunately for me, my BMI is well over 35, and I have Type II diabetes, degenerative joint disease, high cholesterol, undiagnosed incontinence, and probably IBS. Are those co-morbidities? LOL
