First You should research Surgeons and take into consideration how close they are to You and If they are one You would trust There are wonderful people on this site who tell abot there surgeons and there opinion of them. Once You have chosen a Surgeon then You call them and see if They take Your insurance, and try to schedule an appointment once You have done that the doctor will probably send You out a packet telling you what you need to do and have ready for Your appointment and I would do this as soon as possible cause these surgeons are very busy and sometimes consults are 6-7 months away....kinda nerve wrecking for us anxious obese!! But that should get You on Your way. I will mail You a copy of the check list I got from my surgeon if You request but I don't know if it is the same as all Surgeons but should be similar. Let me know. Luck

Because my insurance was an HMO, I had to get a referral from my primary care physician. His office is also where I had my preliminary blood work done (cholesterol, thyroid, etc.). His office took care of hooking me up with a surgeon. Of course, I looked into the one they chose. I didn't like the first guy's record, so they selected a different surgeon for me and I was quite satisified with him because of his amount of experience. Anyway...the surgeon's office sent me a mountain of paperwork to fill out. Once it was returned, I was on a waiting list for a consultation. After the initial consultation, I had to wait again for insurance approval, then my surgery date was set. My very first visit to my primary care physician was in early Sept. 2003 and my surgery was end of May 2004. I could have scheduled it earlier but because I am a teacher, I wanted it done as close to the end of the school year as possible. It took me 9 months from first referral to surgery, but like I said the surgery date delay was my preference, so it might not take that long otherwise.
