Give Dr. Bolar's office a call in Lexington. They are listed as Bariatric Institute of Kentucky. Not sure of the number right now. They'll at least go on and give u an appt date. And Dr. Bolar is in my opinion the best. I know SEVERAL people including myself who he has done. Good luck, Jennifer G.
