If it is Kentucky Medicaid, I do know that it requires a 6 month supervised diet not sure about the other requirements cause I don't do Medicare or Medicaid. But at the seminar I was at the other day the director of the Bariatric Center stated that KY Medicaid requires a 6 month supervised diet
