6 month diet/form question PLEASE HELP

I am just starting the whole 6 month diet and nutrition program that oh so many insurances require. I am going to make an appointment with my physician but I am unclear to as what information the physician needs to complete to satisfy the insurance complanies desires. My physician doesnt seem too excited about completing any forms but she said if I would supply the form she would do it. Anyone have a copy of the form they used or know where I could get one? I already talked with Dr Husted's office (surgeon) who couldnt supply me with one either.  Im feeling a little down and out about the whole process. Sigh. Any info would be helpful. THANKS

From what I understand your doctor needs to note on your medical record your weight and what you are doing to loose weight, what he recommends and these need to be documented EACH of the 6 months in order no skipping a month at all. BUT that being said you should probably contact your insurance and find out exactly what they want as a friend of mine has an insurance company that wants a medically supervised weight loss program...and it was just out of her budget. The insurance company MAY have a form for you.

Hey Shea! Good to see ya again. Congrats on getting your process started. At the drs office I worked in, we would give the patient a copy of that days notes before the patient left. It included your current weight, weight at last appt, what type of diet & exercise you were doing and any questions/concerns you were having as a result of said diet/exercise. I dont think there is a standard form as we dealt with many insurances. My husbands dr gave him his notes at the end of the 3 month program but we gave ours after every visit...dont guess it matters.

Hi Tracy! Good to hear from you too! Yes I am back on the weight loss journey (after gaining 30 pounds since I was here last) sigh! Anywho, its now time to get down to business! I attended a WLS seminar and sent in my package, both of my insurances have been contacted and now I just have to do the 6-7 mo diet and all that. I sure hope my 6-7  months goes as fast as everyone says it does. I appreciate all of your info as well as everyone else that contributed! Thanks

Shea,

I got a copy of my doctor's chart every month and faxed it to Dr. Rod's office.  I'm sure they sent it on to my insurance company.  The best thing would be contact your insurance company and see exactly what they require.  They should be able to tell you.

i just finished my six months and i had to send in a year of my medical records with the last six months listing my weight, bmi, blood pressure and what we discussed as ways to improve my weight loss journey.  it's very important that you dont miss an appointment or you have to start all over.  good luck and trust me the six months really does fly by..mine was over before i knew it..
