What info do I need to keep track of when I visit my primary doctor for the 6 month diet?
I read either on the KY board or the lap band board where someone posted what information to keep track of from your monthly visits with your doctor while doing your six month diet/exercise program. I thought I printed it off but didn't and now I cannot find the post. I called my insurance company and they said the usual, height, weight, blood pressure, exercise. Seems like the post I cannot find had some other information listed. Can anyone help me out with what else I should keep track of? Thank you, Donna B.
I let my pc know that I need documentation of my monthly visits, weight, and any suggestions she had. At the end of the 6 months she filled out a form that asked for my weight @ each visit, and any notes she had. She commented on my wanting to lose weight and that I was trying. Hope this helps. Oh yea, I was told by my wls that I couold not gain while on the 6 month diet but I did not have to lose. Check with your wls to see what rules you must follow to qualify.
All of the above plus make certain your PCP documents your BMI at each visit. Need to document specifics of diet....following weigh****chers, reducing calories, low carb/high protein etc. Also the exercise piece is important. My surgeon's office is a Center of Excellence so they helped out a lot as did the hospital program.
