Amanda- I live in N Cen KY and when I looked the nearest is in IN or TN, and I am not 100% certain of the TN doc. I do not know about the insurance.  I do know that if the website says they do revisions....make sure you double check before you bother with an appt often they are wrong.  IF they do revisions most do not - check if they do to DS.   My biggest concern is the amount of time you would need to be there, and follow up (both of which are medically necessary and completely understood).  The female doc, Margaret Ingram  in IN is supposed to be absolutely wonderful. Once you have completed the actual surgery and basic followup and get into longer periods of time between appts they will let you meet with your PCP and do labs with them. IF there was a DS surgeon here I would cetainly be more willing to attempt it. GOOOOOOOOOOOOOOOd Luck
