Louisville area psych eval?

I am getting my surgery in Ohio, but need to find a Louisville area psychiatrist/psychologist to do my eval before my Jan. 28th consult. I really would prefer a professional who has done these evals before so I don't have to teach them :) If anyone is willing to share who did theirs, please post here or message me. Thanks so much!
