Hello everybody my name is Richard and I am going to have Gastric Bypass by Dr. Shina, I guess the reason I am posting this I am wanting to find out, what in next in line for me.  So far I of course been to his seminary and then I have been to a dietition and pschology, they have me schedule for Pulomary Doctor and Cardio Doctor for Sept,28.  I am wanting to know what is going to go on in their office and also Dr. Shina said he would prefer me to have the filter installed and I am sort of concern what is going to happen then also.  So if anyone can offer me any ideas of what I am about to face in future before the surgery please let me know.   The suspense is killing me.

I would like to know more also. I, too, am going to Dr. Shina, and I have my consult on Tuesday morning.  I am excited, because my insurance company have said that they usually have approx. a 48 hour turn around on approvals......meaning, once they get the papers faxed in from the dr's office, they reply within a 48 hour time frame.  According to the office assistant at Dr. Shina's office, I could possibly have a date by the end of next week!  That would be awesome, since I have dreamed of this moment for YEARS!  Any added info would be great!

Hey Consult is nothing he will hear his staff talk and they will call you one by one to get weigh and photo done.  Then they do a talk about all the paper work and it lasts about  a hour or so.  Then the doctor comes in and talks to you it last about two hours or so or until he has answer all of the questions. Pay attention to him talking he is very interesting and plus he gives you alot of knowledge and things to think about .  Then they will call you back one at a time for the doctor to talk to you personal one on one with him.  At the most that takes about 15 minutes at the most with that and that is the end of the senimary. And you are done as with that part of it.  I hope this help you on this.

Hi Richard! ....I first wanted to say congrats on your decision to have this surgery! I am also a patient of Dr. Shina's, my surgery is set for Oct 1!!!! Dont let the suspense kill ya lol, it about did the same thing for me, I was on here "and still am" asking tons and tons of questions. The replies were awesome & very helpful "Thanx again everyone". I had my seminar on the 12th of June with Dr. Shina and within a month they had my Dietician & Nut set up along with my Cardio and Pulmonary appts... At my cardio all they did was ask me questions and did an EKG. My Pulm appt they asked questions, did a chest exray and listened to my lungs. All went well & smoothly! Did they tell you "Dr. Shina's people" that you need to do a sleep study? I had to. All my appointments came & went very fast "even though it seemed forever to me lol"... I got a call on Aug 12th telling me I was approved and ofcourse I got my date for Oct 1. As for the filter I was'nt told I needed one, it was my choice so on the 19th I get too see about having one put in. Lastly I just had my preops done this past Thursday, they took my blood, did another EKG,did an ultrasound of my Gall bladder, gave me a spirometer too practice my breathing " you have to bring it back with you on the day of surgery",  & there is a class you sit thru after your preops dont miss it, they said you have to sit thru it before they will do surgery. It was very informative & you get to ask alot of questions if ya want  ..Dont let the suspense get too ya, this site offers a wealth of information, along with the members on here. I am drawn to this place 3 or 4 times a day reading profiles, blogs, posts, forums, really basically everything I can get my hands on!...Good luck too you.......Toni

Hey Thank you for the reply it sure is a big help Toni.  Hey Good Luck in the surgery also but also let me know about the filter if you have to have one or not.  Dr. Shina told me I didnt have to have one but he would prefer for me to have it. So please if you could let me know what goes on with that also.  Well at my first appointments they mention to me if I wanted to take the sleep study or did the Dr. said anything to me about and which he didnt mention. So I tol them we will let him decided but as for now I wasnt going to sign up for that.

Your very welcome....He told me I didnt have to have it either, that it was up to me if I wanted it. I decided that I did want it and they set me up for the 19th as I mentioned before. I will let you know what goes on when I find out ! As for the sleep study the nurse asked me about it when I went to see the Nut doc and Dietician. They even went as far as setting up an appointment for me which was nice of them, I ended up calling Dr.Shina's office after I got out of there and I was told not too go to that appointment, so I cancelled it. When I went to the Lung doc he was the one who said I needed it and they set it up for me.. I called Dr.Shina's office back and was told too keep it...It really was no big deal in there end....Well I am going to get off of here... I'll catch up with you soon.. Have a good night .............

What is the filter for? I don't think I have heard much about a filter, or its purpose.

It's called a Vena Cava Filter....

A vena cava filter is a device inserted into a major vein to prevent a blood clot from entering the lungs.

Purpose

The purpose of a vena cava filter is to prevent a blood clot from potentially traveling to the lungs. A thrombus clot traveling to the lungs is called a pulmonary embolism (PE). A thrombus in the deep venous system (the part of the circulation that brings blood back to the heart) represents a disorder of normal hemostasis (the normal clotting of blood).

Insertion of a vena cava filter is indicated for patients who:

  • cannot receive medications that can dissolve the clot (anticoagulation therapy)

  • have a thrombus in a deeply situated vein

  • experiance complication of anticoagulation therapy such as bleeding

  • experiance failure of anticoagulation therapy to prevent pulmonary embolism

  • have an embolus in the lungs (pulmonary embolectomy) removed

  • have a recurrent embolism while receiving adequate medications

  • have significant bleeding complications during anticoagulation

Thanks for the reply. I haven't heard of these, so this helps me understand completely. Do all WLS patients require them, or is it due to certain medical conditions?
