I believe this is pretty standard. At the office where I went, you paid half $300 up front and the other half when they scheduled surgery. Sherryl

Our program fee was for the nutritionist and our psy evaluation.  Plus we had classes that were manditory.  These charges for us was well worth it.  We didn't have to go all over for these other visits you need prior to surgery.  I wished it would have been covered or at least filed to insurance but they probably got burned so many times with insurance. That is why they make it an out of pocket expense for the patient.  I too paid for mine in payments.  Just had to be paid in full by the time of surgery.  Good luck Jane
