Got Anthem BCBS HMO?

I am looking for info on approval by BCBS HMO. Does anyone else have it?

Hi, this is my first reply. I have BCBS Anthem. To get started I went to LivLite Weight Loss Center. They called BCBS for me and told them what the criteria was for Bariatric surgery. I have to do a supervised 6 month weight loss program with a dr and a nutrionist (I will be going for my 5 month checkup Wed) You have to be at least a 100 lbs overweight have a BMI of 40 or comorbidies (diabetes, high blood pressure, sleep apnea, depression, joint pain) Which I do have 4 of these that I know of right now. After the 6 month weight loss program LivLite will send in all my information to BCBS and then they will decide if I qualify for the surgery. So I do know that you need to contact them. Which I did also after LivLite did. They will tell you all this information. Don't be afraid to call them. That's what we pay them for. So far they have paid for all my dr's visits. Minus the co-pay.
