Need help with appeal letter!!

I just got my approval for my breast (pleasantly unexpected) but was denied panni. My doctor is going to write another letter of necessity explaining complications in greater detail but I need to write a letter also. If anyone cares to send me sample letters, I would greatly appreciate it. I'll be leaving for work soon so I'll not be checking in again until late this evening. I posted this on the ps board and am posting here because I don't post often but I lurk daily and I know you guys give great advice and lots of support to each other. My insurance is BC/BS of Minnesota. Thanks ahead for your help.

Yeah, I just finished reading her story on her web site and I really enjoyed learning her story. She really is a smart cookie and does offer a lot of useful information--I got her in my favorites! Dee
