Anthem PPO...new ins for me..help:)

Ok I have Humana PPO on my hubbys plan and after customer service lied to me and the doctor come to find out it's an exlusion. Well i started a new job and right now I can get Anthem PPO ..Care-focused 330. I called them and they stated that "WLS is covered as long as it's medically necessary"..and that I meet the criteria. What is the criteria for Anthem? Also, do you know if you have to have the six month dr supervised diet with them? Basically my file is at the doctors office ready to go for this insurance ...if I decide to enroll with it at work. Need advice...

I have anthem ppo, I did have to show I saw my pcp 6 times in the last year and talked about weightloss. My BMI was only 36, but I had 3 comobities. I was approved once everything was sent to insurance in 4 days. Hope this helps.

Lola i sure hope your rite abut the inc, i called them they told me they dont pay for it, i asked even if it were a madical neseary, maybe i dont have the same as you , anthem bc/bc access ppo! is this what you have? Please let me know, Thanks Delilah!

Thats what I have. Maybe it is something with your company. I would ask your surgeons office. Mine knew right away what I needed for approval.
