Trying to write my insurance letter......
I have copies of my medical records from 1994 to the mid 2003's. I do because I had problems with the PCP's I had and started getting copies of all my records. Anyway I went through them and this is part of what I've got written for my insurance letter. The Doctor supervised diet I was on started back november of 99. But it did last 6 months. And it shows as soon as I went off of it I went right backup and then some. Do you all think this would work? Thanks. Kathy
8-19-1998 requested dietary referral--- Humana denied. Dr. Jerry Jamison at this time I weighed 259
11-10-99 Started physician guided weight loss program with Dr. Gregg Lee. Was put on Fastin.
Starting weight 259 11-10-99
244 02-16-00 (15 pound loss)
228 04-18-00 (27 pounds loss total)
224 06-21-00 (31 pounds loss total)
This diet lasted 6 months.
My weight since then by date at doctors visits.

***Topamax was introduced 2-19-02 by Dr. Stapp as not only an addition to treatment for my depression but as a medication that she hoped would help me with weight loss. Prozac was also used in hopes that I would see some weight loss benefit.

You've done a good job documenting your efforts over the years, and that might be valuable for your surgeon's office or for your PCP. But before you do much more work, you might want to find out if you even need to write a letter. (I didn't, although I needed letters from my PCP and my orthopedist.)
You can call your insurance company to see if they need (or want) a letter from you. They should be able to tell you exactly what kind of documentation is necessary for them to approve your surgery.
Also, maybe you'll get some of that information at the seminar you're going to. If they don't cover insurance, you might ask whom you can contact at their office for advice.
You don't want to spend too much time on this if it turns out to be unnecessary or if the insurance company won't even look at it.
I fretted over this and even wrote a draft of a letter, but then I learned that my insurance company wouldn't even look at a letter from me! It was good therapy for me to look back at my diet history (as I'm sure it is for you too), but you might put it on hold until you learn if it's worth your effort. Save all of that energy and excitement! Once you've attended the seminar, you'll have a better feel for what you need to do.
