Has anyone been approved through Preferred Health Professionals-BAC Local 15

I was recently denied surgery and was wondering if anyone in the KS area has this ins. It is a self funded plan through the Bricklayers and Allied Craftworkers. I would appreciate any info of anyone with experience dealing with this health fund. Thanks!
